Fatal work at height incident

An aging telehandler fitted with an even more questionable platform overturned earlier this month in West Philadelphia killing one of the three occupants and seriously injuring the other two. The three men were brothers.

The incident occurred on September 6th, while the three were apparently working on installing new air conditioning units at a height of around 15 to 16 metres when the telehandler - a 17 metre Pettibone 10056 - tipped over sideways, with the fully extended boom coming down onto a parked van.
One man, Tuan Vuong, 47, the eldest of the three, was totally unresponsive and died en route to the hospital. The other two were listed as in an extremely critical condition. One of the two, Wei Vuong, left hospital on September 13th, while his brother Nguyen remains in a coma. The three men worked for the family company - Vuong HVAC which was contracted to carry out the work by Smart Builders. Tuan Vuong leaves his wife and three children ages one, six and nine.
